It chronicles the entire person's health journey using both an in-cloud portal and a Patient Engagement Tool, to connect patients and caregivers at point of care, reducing critical health events for a longer residency at the retirement home. Its secure, connected Regional Digital Healthcare Eco-system integrates with Retirement Homes, EMRs, insurance and government data assets for an all-encompassing view of patient records.
Health Espresso empowers health organizations with Digital Front Door with optional Seniors, Mental Health and/or wound care assessments as required. A digital Care Plan is simultaneously created for multi organizational collaboration for better patient experience and healthcare outcomes, followed by system Navigation reducing administrative work.
Wound care: Optional post-discharge Tele-Monitoring (metrics and wounds) is integrated for better health results.
Telemedicine: Embedded government (OTN) certified Secure Messaging and Video tool connects practitioners and patients for collaborative consultation resulting in informed decisions at point-of-care.
Virtual Care: In partnership with iCARE Virtual offering Virtual In-House Medical care, Health Espresso's Patient Engagement Tool complements its in-cloud web portal to empower physicians with real-time patient information and virtual care capability for time-sensitive decisions at the point of care.
